Transition planning is about equipping young people for adult life.
Young people are encouraged to be part of the planning process, they work with the Connexions PA and our Inclusion Manager, Stuart Crooks, to prepare for their person centred Education Health Care Plan, (EHCP), Meeting. From the age of 14 the EHCP meeting includes a transition outcome to prepare pupils for moving on. The transition outcome may consider daytime activities, living arrangements, leisure opportunities, health care and learning needs such as increasing independence and experience of other providers.
During the transition years there is an increasing emphasis on experiencing the world of work and enjoying links with other environments including schools, colleges, day care providers and community facilities such as libraries, leisure facilities and shops.
Transition can be an anxious period for everyone but the school will help to signpost families to other organisations offering support.
